HIPAA Notice of Privacy Practices

Your information. Your rights. Our responsibilities.

This notice describes how medical information about you or your child may be used and disclosed and how you can access that information.

Your Rights

You have rights regarding your health information. These rights may be exercised by you or, when applicable, by a parent, legal guardian, or authorized personal representative.

Get a copy of your health and claims records

You can ask to see or receive an electronic or paper copy of your medical record and other health information Compassion Pediatrics has about you. Compassion Pediatrics will provide a copy or summary, usually within the time required by law, and may charge a reasonable cost-based fee when permitted.

Ask us to correct your health information

You may ask Compassion Pediatrics to correct health information you believe is incorrect or incomplete. Compassion Pediatrics may deny the request in certain circumstances but will explain the reason in writing when required.

Request confidential communications

You may ask Compassion Pediatrics to contact you in a specific way, such as at a particular phone number or mailing address.

Ask us to limit what we use or share

You may ask Compassion Pediatrics not to use or share certain health information for treatment, payment, or operations. Compassion Pediatrics is not required to agree to every request, except where the law requires otherwise.

Get a list of certain disclosures

You may request an accounting of certain disclosures of your health information for the period allowed by law.

Get a copy of this notice

You may request a paper copy of this notice at any time, even if you agreed to receive it electronically.

Choose someone to act for you

If you have given someone medical power of attorney or if someone is your legal guardian, that person may exercise your rights and make choices about your health information as permitted by law.

Your Choices

For certain types of information, you may tell Compassion Pediatrics your preferences about what is shared. Where applicable, you may have choices regarding sharing information with family or friends involved in your care, disaster relief, fundraising communications, or other situations described by law.

Compassion Pediatrics will obtain written authorization before using or disclosing health information for purposes that require authorization under HIPAA.

How We May Use and Disclose Health Information

Treatment

Compassion Pediatrics may use and share health information to provide, coordinate, or manage treatment and to communicate with other professionals involved in care.

Payment

Compassion Pediatrics may use and share health information to bill for services, obtain payment, verify coverage, and manage insurance or benefit-related activities.

Health care operations

Compassion Pediatrics may use and share health information to operate the practice, improve care, train staff, conduct quality activities, and manage business operations.

Other permitted or required uses

HIPAA and other laws may permit or require additional disclosures, including for public health and safety activities, health oversight, workers’ compensation, organ donation, law enforcement, judicial or administrative proceedings, certain research activities, and other purposes authorized by law.

Our Responsibilities

Compassion Pediatrics is required by law to maintain the privacy and security of protected health information.
Compassion Pediatrics must notify affected individuals when a breach of unsecured protected health information occurs as required by law.
Compassion Pediatrics must follow the privacy practices described in the notice currently in effect.
Compassion Pediatrics will not use or disclose health information other than as described in this notice unless you authorize it in writing, except where otherwise permitted or required by law.
You may revoke a written authorization at any time in writing, subject to actions already taken in reliance on that authorization.

Questions or Complaints

If you believe your privacy rights have been violated, you may file a complaint with Compassion Pediatrics. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights.

Compassion Pediatrics will not retaliate against you for filing a complaint.
